The Care Forum is celebrating the launch of their new well aware website which will enable professionals, community leaders and residents to access quality assured information on support and wellbeing services. The move will allow The Care Forum and its team to continue their work within the community and to ensure that quality data information is readily available to people when they need it most.
The launch comes as part of innovative partnership which has been working to establish best practice in data quality assurance. This process will ensure that Bristol and South Gloucestershire residents have pertinent up to date support information at their fingertips.
During the pandemic, there has been a sharp focus on health and social care services, and The Care Forum has played a vital role in ensuring that resident’s voices are heard. This new website will continue this trend by ensuring that quality data is shared with other partners in health and care and clinical settings to ensure a systemised approach to referring patients accessing primary care and social prescribing.
The work of our Well aware team, in Partnership with the Bristol City Council, South Gloucestershire Council, BNSSG, Place cube and our long term collaborator Doc&Tee means that people can get data that is clear, concise and standardised in one place. Whether it’s a referral for social prescribing or as a resident you simply want to know what’s going on in your area on the weekend, this service makes finding quality data simple.
